
It is a requirement of the Ontario Baseball Association & PDMBA, in line with Canadian Legislation, that all coaches and individuals directly involved with players complete a Screening Disclosure Form and obtain a Police Check in compliance with the Baseball Ontario Volunteer Screening Policy.
If you do not currently have a valid police check, you must arrange to obtain one as soon as possible as they take 2-3 months to get back from the date of submission to the OPP.
As we are a not-for-profit organization, we can provide you with a document that allows a police check to be carried out free of charge. To obtain this document, please download the VSC Letter to the Police Document ( Click Here) . Once downloaded, complete the form, then scan and upload the letter to the OPP site. The link is shown below:-

Follow the online prompts. You will be applying for a Vulnerable Sector Check. The last prompt on the OPP Form will ask if you are applying for a police record check within Canada. The answer to this question should be YES.
Once you receive the results of your police check by email, you MUST complete the PDMBA Screening Disclosure & Police Check Results Form. This Form will allow you to attach your Vulnerable Sector Check (Police Check) so it can be kept on file with PDMBA.
If you have already completed your Screening Disclosure and Police Check Result Form (Vulnerable Sector Check has been attached and is in the PDMBA system), you only need to complete the Police Check Declaration Form as long as your VSC was completed within the last 3 years.
All coaches and volunteers who are on the field with the players have to fill out similar forms with Baseball Ontario
You are required to complete:
1) Volunteer Screening Level 1
2) Safe Sport Module through CAC
3) Volunteer Screening Level 2
Please note that Volunteer Screening Level 1 and the Safe Sport Module through CAC need to be completed before you can "enroll" in Volunteer Screening Level 2
