Photo Consent:

I authorize Port Dover Minor Baseball and/or its staff, associates, assistants, or subcontractors to use photograph(s), video and other media image provided for any Port Dover Minor Baseball  promotional, educational or other pertinent uses. I also agree to Port Dover Minor Baseball using any images of the named minor(s) gathered by photographers, and or videographers supplying images to Port Dover Minor Baseball . I understand that some photographs with names will appear in local press publications for example when reporters attend games, practices or other events together with Annual Award Ceremonies.

These images may include, but are not exclusive to, club, coach, athlete and/or parent, and/or volunteer submissions by various member associations of Port Dover Minor Baseball.

I authorize Port Dover Minor Baseball to permit the use and display of photographs and/or recordings of the named minors in any Port Dover Minor Baseball  publication, multimedia production, including video and web usage, display, or advertisement.

I agree that Port Dover Minor Baseball may use name, likeness, or information supplied by the undersigned.

The undersigned releases and forever discharges Port Dover Minor Baseball and its officers and employees from any and all claims and demands arising out of or in connection with the use of said photographs / recordings, including but not limited to, any claims for invasion of privacy or defamation.

Dual Registration:

By completing this form, you are registering with both Port Dover Minor Baseball and Baseball Ontario.  Baseball Ontario appoints Port Dover Minor Baseball  to collect fees on its behalf, included in the registration fee collected through this process. No additional monies is charged to the parent registering locally. Baseball Ontario invoices Port Dover Minor Baseball for fees collected.
Liability Waiver:

Waiver for Main Season Registration, Tournament Registration, Winter & Fall Ball Practice.


On behalf of myself and the player being registered, I recognize that baseball entails serious risks. Consequently, except as noted below, I and the player being registered relinquish all rights to a claim of any kind, including the right to a claim for bodily and material damages, regardless of the cause, against Baseball Ontario, its member associations, including Port Dover Minor Baseball  and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors, even if such damages result from negligence of Baseball Ontario, its member associations, including Port Dover Minor Baseball  and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors.Without restricting the generality of the preceding, I and the player being registered also relinquish the right to any claim against Baseball Ontario, its member associations, including Port Dover Minor Baseball and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors resulting from a decision on their part, regardless of the nature of this decision.This release of Baseball Ontario, Port Dover Minor Baseball and the other persons noted above does not preclude myself or the player being registered from making a claim under any sports accident coverage provided by Baseball Ontario and/or Port Dover Minor Baseball to their players. An overview of  the current sports accident coverage provided by Baseball Ontario can be found on the Baseball Ontario Website in the Insurance Program Overview.

Refund Policy

Port Dover Minor Baseball Refund Policy

All Refunds are subject to a $25 administration fee

Note: For the purpose of this policy, the Year-End Tournament Fee refers to any end-of-season tournament, series, playoff, playdown, or similar event that takes place after the regular season.

100% Refund of Player Fees and the 2026 Year End Tournament fee, (does not include Administration fee), if notified in writing by and including April 1st (email sufficient to [email protected] For the Attention of the Treasurer)

75% Refund of Player Fees and 100% of the 2026 Year End Tournament fee, (does not include Administration fee), if notified in writing by and including May 1st (email sufficient to [email protected] For the Attention of the Treasurer)

50% Refund of Player Fees and 100% of the 2026 Year End Tournament fee, (does not include Administration Fee), if notified in writing by and including June 1st (email sufficient to [email protected] For the Attention of the Treasurer)

No Refunds given after June 1st

Any refunds requested due to an injury making the player unable to play or moving to another association, will be dealt with on a case-by-case basis. Please contact (The President of PDMBA on email. Please go to contacts for the address)

Registration fee adjustments due to Covid and/or unexpected occurrence related changes:

Registration fees may be adjusted/refunded should the Season be impacted by Covid-19 or other unexpected occurrence. The Executive will discuss and decide on the way forward in this circumstance and inform all affected players.

This policy covers all players fees for each team

Baseball Year-End Tournament Refund Policy

Effective 2026 Season

Definitions

· Year-End Tournament Fee: A fee paid by all players at registration to cover SMBA/SCBA year-end tournaments, playdowns, or playoff series. OBA Tournament Fee: A fee paid by Rep players only for the OBA Rep Provincial Tournament.

1. Year-End Tournament Fee (All Players)

Beginning in 2026, a $50 Year-End Tournament Fee will be collected at registration. This fee covers year-end tournaments, playdowns, playoff series, and other end-of-season activities.

This fee has historically been collected at season end; collecting it at registration simplifies administration and ensures equal contribution from all players.

The Year-End Tournament Fee is a team-based, non-refundable expense. Refunds will not be issued for players or teams unable to attend year-end events.

2. OBA Tournament Fee (Rep Players Only)

Rep players will be charged a $50 OBA Tournament Fee. This fee is non-refundable, except as outlined below. This fee is in addition to the Year end Tournament fee (Example: SCBA Tournament).

3. OBA Tournament Fee – Refund Eligibility

The OBA Tournament Fee will be refunded to rostered Rep players only if:

· The team formally declares before the OBA deadline that it will not participate; or

· The team declares intent but is eliminated through playoff playdowns and does not advance; and

· The association is not charged an OBA entry fee and not fined by the OBA.

4. OBA Tournament Fee – Non-Refundable Situations

No refunds will be issued if:

· The association is charged an OBA tournament fee due to team declaration and withdrawal

· The association is fined by the OBA due to OBA rules, deadlines or procedures that are not followed

5. General Statement

Refunds will be issued only when all rules are followed and no fees or fines are incurred by the association. Any fees or fines resulting from team actions will make tournament fees non-refundable.


Rowan’s Law:

Under Rowan's Law, before any player can be registered with the local association and Baseball Ontario, the player, and the parent or legal guardian of the player if the player is under 18 years of age, must review the Concussion Education Resources provided by the Province of Ontario and must review the Player Code of Conduct. Rowan's Law Acknowledgement:

I hereby confirm that the player being registered to participate in baseball activities with the local association and Baseball Ontario, and the parent or legal guardian of the player being registered if the player is under 18 years of age, have reviewed the concussion education resources provided by the Province of Ontario as referenced above and have reviewed the player code of conduct as referenced above. I / we further acknowledge understanding the nature and risk of concussion and head injury to athletes, including the risks of continuing to play after a concussion or head injury is suspected.
