Once Completed it will be automatically Emailed to the Awards Ceremony Manager

This form will inform the Award Ceremony Manager of the number of tickets required to attend the End of Year Award Ceremony.

It is essential that you enter the Players name and the team they are associated with

The form allows for up to three players. If any further players are required please indicate in the Comment box the extra players name and team they are associated with.

On completion of the form you will be emailed with a copy of your ticket request which will be proof of your intended attendance

$12 for each admission to the Community Centre
( Same price for all Players, Non Player Child and Non Player Adult attendance )
