COVID SCREENING AND CONTACT TRACING REQUIREMENT

Please Select the team you require from the list on the left of the screen.

Selection of each team will present toward the top of the page on a black banner:

"Click here to complete a Digital Health  Screening"

Once clicked you will be invited to sign on the Sportsheadz application.

If this is your first visit just use "Click here to Register" Button. Otherwise use "Click here to sign in" button. This application will ask you to select the event date and time that you are attending and then ask you to confirm your attendance. IF YOU ARE ON THE DAY OF THE EVENT the system will provide a list of Covid questions. If you pass you are allowed to attend the Practice/Game.

Stop at Stop sign on entering Misner Park Port Dover.
OPP monitor and will issue tickets

Do Not Park around Pavilion area unless to manage team equipment
